2. Your spouse's Social Security number, date of birth, occupation and if military, your home of record.
3. Your home address, home phone, work phone, and cell phone and e-mail if applicable.
4. Do you own the home you live in?
5. Did you buy or sell a home in the current tax year?
6. Do you own your own business and if so what kind?
7. Did you sell, swap, trade, or exchange any stocks, bonds, mutual funds or unimproved property and if so, do you know the purchasing price of said sales?
8. Do you have any rental property and if so what kind?
9. Did you pay someone to watch your children while you worked?
10. Did you make any payments on a student loan?
11. Did anyone in your family attend college or tech schools in the current tax year?
